This form is the first step to enrolling your new student online. Complete it to request an account that you will use to log in to a secure system.

If you have another student already in Dickson County Schools, you will use your Skyward Family Access account. Please contact your school staff if you need a reset link to your account. Complete required fields to request an account to enroll your students. Legal name, phone number, and e-mail are required. If you do not have an e-mail, please visit https://support.google.com/mail/answer/56256?hl=en to create a free gmail account. If a student is experiencing homelessness or is an unaccompanied minor, contact Dr. Mary Collins, the Homeless Liaison (615-446-7571), for assistance with the enrollment process. Please provide the school or upload the following: Please attach a copy or picture of each document to return with this enrollment form. _____ 1. PROOF OF CHILD'S AGE (Birth Certificate, Mother’s Copy, religious, hospital, or physician’s certificate, adoption record, etc.) _____ 2. TENNESSEE DEPARTMENT OF HEALTH CERTIFICATE OF IMMUNIZATION FORM CHECKED FOR YOUR GRADE LEVEL. _____ 3. COMPLETE, CURRENT PHYSICAL EXAMINATION (Only if first time enrolling in Tennessee school) _____ 4. THREE PROOFS OF RESIDENCE (home ownership proof, rental agreement, driver's license, voter registration card, current utility bill, cable bill, etc.) The following documents are requested for certain situations, if applicable. _____ 5. ANY CUSTODY PAPERWORK (Power of Attorney is not allowed. Paperwork must be a court order with a judge’s signature.) The entire document needs to be provided to the school, to keep on file. _____ 6. IEP for students with a disability. The entire IEP, if available, is necessary to ensure correct placement for grades K-12. The IEP documentation is also for PreK selection. _____ 7. PreK Only: TIER DOCUMENTATION as noted on the Voluntary PreK District Application and/or INCOME ELIGIBILITY FORM _____ 8. Previous REPORT CARD or TRANSCRIPT from your previous school
